Case File Documentation Requirements
- Valid identification document is required for case file creation.
- Original National ID Card
- Original Birth Certificate
- Passport
- Having a supplementary insurance booklet from a provider contracted with Parsa Hospital
- Referral letter from the insurance company contracted with Parsa Hospital
Hospitalization Documents
- Hospitalization order from the attending physician
- Having a valid identification document to complete the patient’s identity information in the admission program and for case file creation as mentioned above.
- Bringing the patient’s supplementary insurance booklet, if they have a supplementary insurance booklet from a provider contracted with Erfan Hospital.
- For patients in the gynecology ward and delivery block, providing a valid identification document of the companion (copy of the spouses’ birth certificates and national ID cards, as well as the home postal code) is mandatory.

To prepare for your appointment with the physician, the following are recommended:

- If you are following a special diet, bring its details.
- Depending on the type of your appointment, ask friends or relatives to accompany you. You may need someone to drive you home.
- During your meeting with the physician, ensure that you ask all your questions and that your concerns are fully addressed. You can see the questions you can ask your physician on the “Questions a Patient Should Ask Their Physician” page.
- After meeting with the physician, if you wish to schedule appointments with other specialists, we will arrange this for you as soon as possible. Also, if you need to be hospitalized, we will make the necessary arrangements for admission to the hospital.
