Discharge Guide and Receipt of Documents
- The discharge of hospitalized patients is available during weekdays from 10:00 AM to 7:00 PM.
- The discharge order will be based on the instructions of the attending physician. The patient’s file will be completed by the section secretary and sent to the discharge department. (The discharge process is based on the duration of hospitalization and the medical procedures performed during the specified time period).
- All charges at the center are calculated according to the Health System Reform Plan and based on the current year’s Relative Value Guide for medical and diagnostic services. The bill will be provided to the patient.
- The medical expenses for patients with contracted supplementary insurance will be calculated according to the agreement with the insurance organizations, and expenses outside the coverage of those organizations will be collected in cash.
- If your patient settles the hospital bill as a self-paying patient upon discharge, but also has supplementary insurance, they can follow these steps to submit the necessary documents to the insurance company and receive reimbursement:
- Call to request medical records at phone number +98 21 53283000, extension 401, two weeks after the discharge date for follow-up on the documents to be sent to the insurance companies. Please contact the provided number before arriving to collect the documents.
Please note that document delivery will be available during business hours (except Thursdays) from 8:00 AM to 2:20 PM.
Important Note:
Documents will only be provided to the patient in a single copy. In case of loss, no duplicate will be issued. Documents will only be handed over to the patient or their immediate family members (father, mother, spouse, brother, or children) upon presentation of valid identification. If someone other than the aforementioned individuals comes to collect the documents, the original national ID card of the patient, along with a consent letter from the patient authorizing the representative, as well as the national ID card, birth certificate, or driver’s license of the person collecting the documents, must be provided. If the patient resides outside of Tehran, for their convenience, the documents will be sent to the address specified on the form.
All patients must attach their discharge bill to their insurance documents. If the patient was referred to the hospital with a referral letter, the hospital’s insurance unit will send the patient’s documents to the relevant insurance company. For information and follow-up regarding the submission of their documents, patients should contact the insurance unit at +98 21 53283000, extension 401, Mrs. Fathi. Any tampering with the contents of the medical file, for any purpose, is considered a criminal act and is a violation of the legal provisions regarding the alteration of official documents, with corresponding penalties defined for such actions.
